noun Hand-over or Handover. A term for the passing of one or more patients from the care of one doctor or nurse to another at the end of a shift or when the patient is to be transferred to another service. Proper hand-over of acutely unwell and at-risk patients to responsible persons on the next shift reduces error and deals with critical issues in a timely manner.
During hand-over, the responsible healthcare worker informs his/her counterpart of tests ordered, management issues and evolving or resolving problems.
