malignant neuroleptic syndrome
A life-threatening condition that affects < 1% of patients exposed to neuroleptics (e.g., phenothiazine, butyrophenones).Clinical findings
Fever, autonomic instability, muscle rigidity, tremor, cognitive changes, encephalopathy, delirium, and myoglobinuria, which start 1–3 days after beginning neuroleptic agents, including antipsychotics or antidepressants.
Lab
Increased CK, leukocytosis.
Management
Stop drugs, antipyretics, cooling blankets, dantrolene, bromocriptine.
