| 释义 | Med 4Med 4A now-retired, green-coloured sick note filled by GPs for workers in the UK unable to work for a specified length of time, or for periods longer than 6 weeks (shorter periods were covered by the Med 3 (up to 2 weeks) and Med 5 (2 to 6 weeks) forms). Med 4s are used to claim incapacity benefit and the patient may be examined by a Benefits Agency doctor. It was replaced in 2010 with the Statement of Fitness for Work. |
