Med 5

A now-retired, pink-coloured sick note that was filled out by GPs for patients in the UK who were too unwell to work. Patients gave the Med 5 to their employers to ensure they would get social security and statutory sick pay when they were sick for longer than the 2 weeks (up to an additional month) or when they had been discharged from hospital. It was replaced in 2010 with the Statement of Fitness for Work.
