Medical Outcomes Study 36-Item Short-Form Health Survey, SF-36A study used to provide perceived health status in eight domains. These domains include:1. limitation of physical activity resulting from health problems; 2. limitation of social activity caused by physical or emotional problems; 3. physical health problems limiting usual activities; 4. bodily pain; 5. general mental health (psychological distress or well-being); 6. limitation of usual activities due to emotional problems; 7. vitality (energy and fatigue); and 8. general health perceptions.The SF-36 is designed for use in those over the age of 14 and is useful in comparing the impact of disease and the efficacy of treatments, and identifying those at risk.
