A complete package of ongoing healthcare arranged and funded solely by the NHS for those it deems eligible, which can be provided in any setting. The care is provided in the place where it is most appropriate per the care plan—if the person lives in his or her own home, the NHS will fund all the care and support that he or she have been assessed as needing; if the person lives in a care home, the NHS contracts with the care home and pays all fees, including costs of accommodation and care.
