A grade of doctors introduced by the UK government in the 1980s as a non-consultant career grade post to make up for the shortfall of doctors in "middle grades", following limitations on the number of specialist registrar posts, in order to find a way of providing essential support to consultants in acute specialities without training doctors for non-existent posts.
