Pernicious anaemia is due to a failure of absorption of vitamin B12 (cobalamin) which is necessary for normal DNA synthesis in the bone marrow and elsewhere. Absorption of B12 requires a factor normally produced by the stomach (the intrinsic factor) and this is absent in pernicious anaemia as a result of an autoimmune inflammation of the lining of the stomach. Pernicious anaemia features tiredness, lassitude, breathlessness, PALPITATIONS, dizziness, PALLOR, a smooth, sore tongue, diarrhoea, a yellow tint in the skin, an enlarged SPLEEN, numbness in the fingers and toes and sometimes DEMENTIA. Treatment is by injections of hydroxycobalamin.pernicious anaemia a severe condition in which there is a progressive decrease in the number of red blood cells together with an increase in their size, producing poor colour, weakness and gut disorders.
