The official investigation that followed the conviction of Harold Shipman, the British GP, for killing 15 of his patients. The inquiry concluded that Shipman had killed no less than 215 of his patients. An important safeguard put in place thereafter was to increase the oversight of doctors who sign death certificates. The Shipman inquiry resulted in a massive surge in the number of post-mortem exams at the level of district general hospitals in the UK. Shipman is the only British doctor to ever have been found guilty of murdering patients.
