CMV infection of peripheral nerves and spinal roots, which is a well-known complication of HIV-1 infection and often leads to acute ascending flaccid paralysis of the lower limbs with areflexia, paraesthesia, urinary and bowel symptoms, generalised weakness and paralysis. Some cases may respond to combined anti-CMV therapy with foscarnet and gancyclovir.
