Form 4 A form once used for appraising doctors in the UK, which is loosely equivalent to the current Appraiser's feedback, based on the appraiser’s assessment of the appraisee's continuing professional development, personal development plan, attribute coverage and any other supporting information at the time he/she views it on screen.Form 4
