Form 656-A

A form one files with the IRS along with a Form 656 declaring that one has paid the appropriate application fee for an offer in compromise, which is a program whereby a person or company owing delinquent taxes asks to settle the debt for less than the full amount owed. If one claims not to owe an application fee (due to low income or other factors) and the IRS determines otherwise, the application for offer in compromise is denied automatically.
