A typically disk-shaped subcapsular lens opacity (OMIM:116600) located at the posterior pole of the lens, which can have a marked effect on visual acuity.
 
Molecular pathology
Defects in EPHA2, which encodes a protein thought to regulate lens fibre cells shape and interactions (and thought to be important for lens transparency development and maintenance), cause cataract (posterior) polar type 1.
