Physiology A 45 kD low-affinity vitamin B12-binding glycoprotein secreted by gastric parietal cells, which closely parallels HCl secretion; IF secretion is stimulated by histamine, gastrin and methionine, and usually greatly exceeds that required for B12 absorption; IF is re-absorbed by specific receptors in the ileum; the high-affinity B12 binder, R protein, attaches to vitamin B12 in the acidic stomach, later releasing B12 to IF after cleavage by pancreatic enzymes; IF is ↓ in Pts with low gastric acid production or by agents which ↓ gastric acid secretion by blocking parietal cell receptors–eg, H2-blockers, but is unaffected by agents that block H+/K+-ATPase-induced gastric acid secretion.
